For teachers (to register teachers and girls):

For scientist volunteers:

  • PAGES Scientist Invitation (pdf)
  • Volunteer Scientist Sign-up Form (pdf) (doc)
  • Affidavit for volunteers that have been Pennsylvania residents for the last 10 years. (pdf)
  • Chaperone Sign-up Form (pdf) (doc)
  • Minor Volunteer Sign-up Form (pdf) (doc)

Please note that a sixth-grade teacher must submit all registrations. Parents may not register their daughters. Each teacher may register up to four (4) girls. The registration form was changed in 2009 to require the signature of a parent or guardian for each girl. Completed registration forms may be returned by FAX, e-mail (preferred method), or U.S. mail. Registration is limited and will likely close before the posted deadline date (typically three weeks before the event), so return the form promptly.

There is no cost to the students. A snack and lunch will be provided for all participants.

Confirmation letters, with maps, will be sent to the teachers about one week before the event. We depend on the teachers to distribute this information to their students and to coordinate transportation for the students.

Parents may not attend the program, except for the keynote address.

Teachers are invited to sign up to be part of the event, too. Teachers will be paired with a scientist to assist them with their experiment. This is a great way for teachers to learn about activities to take back to their classrooms.
